Subject: Dr Gian,
I am a married 51 year old woman who has been married
for 10 years. He is the only man I have been with for 12 years. My
husband told me he has genital herpes after we fell in love and had been
to together for about 6 months. He explained everything, that the doctor
had told him, to me about the disease. Very seldom did we have sex
during a breakout, but we always used protection. For about a year now I
have had several episodes of a small blister like spot come up either on
the side of my vagina or between the opening and my rectum. At first I
thought it may be an ingrown hair but this last time it spread to both
sides. I discussed this with my husband and he was confident I did not
have Herpes. He said that I didn't have classic signs. Pain,
headache...etc. He has not been to the doctor for this since we have
been together. I researched this online and read that Herpes can be
spread even when there is not a breakout at the time of sex. I also read
that lower back pain is a sign, which I have had for about a week now.
What do you think I should do?
Hi,
I think you are right and
should further discuss this with your husband and see your doctor and
get tested. If you are positive also get info about suppressive
treatment..
In good health

Subject:Hi,
I have not had sex for about a year with a partner but now recently I
have been noticing that I have small white or flesh colored bumps around
my penis and on my balls and I have also noticed I got a red rash around
my neck and chest after I masturbated and I am really worried about it
now because I have also been noticing bumps on my back and shoulders
that itch and when broken form a scab. I thought it was some form of
body acne but now I know its not and I am scared because I think it
could be syphilis or Hiv/Aids what do you think?
Hi,
the bumps you describe suggest genital warts and the rash around your
neck may be a Tinea versicolor which is a relatively common skin
infection caused by the fungus Pityrosporum ovale, a type of yeast. And
the bumps on your back? Well consider......herpes type 1? Should you test
for HIV and Syphilis? Yes but also see your Physician.
Be well.

Subject: Post swelling in labia minora
had swelling in one fold of labia minora
( a pimple like bump that burst out eventually) As per doctors it
is folluculitis and I was advised to use great hygine care. Now I
find this fold is increased in size compared to the other side fold
as has been so since last 6 months. It is especially uncomfortable
and slightly painful when wearing fitted clothes.
Please advise is this temporary or will
this go. Is there any medication to cure this?
Appreciate your time and help!! Thank
you
regards
Hello,
I
would report this to your Doctor. It looks like the infection has
persisted in the layers below the skin and mucosa. Medications
definitely help and sometime a lancet to help the drainage. Keep an eye
open to other possibile diagnosis if you have recurrences.
Be well.

Subject: chronic/recurrent B.VMy history
is this: 39 yr. old married x4 years/monogamous G2P2
female with IUD for contraception for the past 4
years...I used to be an ob/gyn nurse...I have had
(cultured positive) recurrent gardnerella B.V. for the
past 8-9 months...i have been treated with Flagyl,
metro-gel, clindamycin as directed each time...the
symptoms subside during and right after treatment but
recur within a week or so...Prior to the iud placement I
had maybe 2 total BV (fully symptomatic) infections in
the course of previous @ 15 years of being sexually
active...I was recently treated with metro-gel for 10
days-twice daily...i was re-cultured about a week or
less after treatment and tested positive for gardnerella
again. Now, my gynecologist is recommending a 5 day
course of metro-gel and to follow up with twice weekly
metro- gel for 6 months...I realize that one should not
have intercourse during treatment--but it seems i am
always being treated...so, it is unrealistic to be fully
compliant with that...I am so frustrated and tired of
being on medication and my husband seems discouraged as
well...My gyne says he does not believe that IUD is part
of the problem, so he does not recommend removal.
Usually my symptoms are mainly some burning type feeling
in the vagina with only occasional classic fishy odor if
it's full blown along with some clearish discharge...I
am a smoker, so no OCP's for me at this point and we are
unsure about sterilization. What are your
recommendations???
Thanks so much for your time!
Hi,
I would grab this
opportunity and stop smoking (shame !! ); I would improve my oxygen and
nutrition delivery everywhere to my body with exercise and other non
antibiotic based food and, finally, I would consider avenues to prevent
pregnancy other than your unfriendly IUD.
In good health,
Dr. Gian
