Previously I took the medicine for 3 months. The dose was 1 + 0 + 1.

Subject: Request Information about HSV

Dear Dr.Gian,
I am zav...from Bangladesh. I am suffering from HSV.
I have tested my blood and found the following:

Hi Zav
What you indicate with your testing results is a Herpes virus type
one infection which has occurred "sometime ago" and has its memory signs
in the presence of antibodies. A not uncommon situation since 70-80% of
people have this type of virus.
Type one is not so aggressive and becomes an issue only when it addresses
the genital area and its relation to sexual activity. If one keeps him/herself
in good shape may not require suppressive therapy on a daily basis. But you need
to help your immune system with good nutrition, exercise and peace of mind.
Acyclovir is a good medication and has a long track record. But it is a drug.
Use it sparingly. Become more keen in noticing the signs and
symptoms (pre-dromal symptoms) which precede a flare up.
Handle ruthlessly whatever stressful situation is occurring.
You will be surprised how many attacks can be aborted that way.
And if you need to use Acyclovir use it no more than ten days.
Do not rely exclusively on Acyclovir. It seems to me you are
on a suppressive therapy program. Try to use it on per need basis
and put yourself to work instead.
How many days will you survive?
You can have a long life, my friend, with Herpes as your companion.
A very long life. You are taking the right medicine for Herpes and do not need other.
Instead look also for alternatives.
Have your wife tested not only with blood test but also with recurrent vaginal
and cervical Herpes cultures. In case of active infection at time of delivery she
could be recommended to deliver with C-section.
Buy this book "Managing Herpes: How to Live and Love with a Chronic STD" by
Charles Ebel and Anna Wald...and remember that you can have a normal and good life even if you have Herpes.

Be well

Dr. Gian

Sir, I do need your help BADLY. Please KINDLY let me as to why I am facing the above mentioned problems.
Best regards -
Zav

I have already responded to your previous email, Zav . I read you additional symptoms
and I am starting to perceive the friendly ghosts which almost always accompany
sexually transmitted disease: GUILT, ANXIETY and RESENTMENT. Could it be this is why you are facing these problems?
The STD's world is universally full of goblins and ghosts, may be because of our lost innocence and authenticity.
Other animals do no seem to suffer so much as we do in this area of diseases. However we can talk.
So let's use it. Talk and talk some more. Find a local good listener and let it all hang out.
See your doctor and see if he/she may want to help you with additional medicines and/or tests.
I doubt your additional symptoms have anything to do with a Herpes infection.

Be well.
Dr. Gian

Subject: Herpes

Dr.
I was recently diagnosed with atypia and they want me to have a colposcopy
my doctor said I have herpes and was wondering if they would have known this
earlier from a pap smear report? Like last year when I was in. Do they check
for STD's and such at that time? I want to tell my ex partner about this so that
he can be aware and not spread it to someone else. Wanting to do the right thing!

Hi,
The PAP test looks for normal, abnormal (cancer) and "iffy" kind of cells.
This last group are called Dysplastic cells or atypical cells. They are cells
which are not normal but not definitely cancerous. Remember, the PAP test is not
a test for STD infections but a screen for cancer. Doctors do not check for STD
unless you have symptoms or first something is wrong with the PAP or you make
a specific request for them. Many studies and my personal medical experience indicate
that when the result of the PAP test shows dysplasia or atypical cells something is going
on in the department of INFLAMMATION of the vaginal and cervical walls. What kind of inflammation.
Well, many kind, but the one that seems predominant is inflammation due to VIRUSES. What kind of viruses?
Numero uno = Human Papilloma Virus, a family of 18 viruses which causes genital warts.
Herpes viruses follows in line with a reduced incidence but with its inflammatory properties
in the cervical area. So when the PAP is not normal ask your Doctor to do a "cervical herpes
and a cervical Papilloma check". When the suspicion is very high Docs do a PAPsure procedure
with vinegar swabs and special chemiluminescent tiny light bulbs and later a colposcopy
and biopsy looking for a diagnosis and a "staging" of cancer. What else. Nothing.
If you biopsy is OK,. repeat the PAP every 6 months 6-8-times and it may come back to normal
and resume the yearly schedule. Dr. Gian's rule number one: Do not ever forget that
an infection is not just an agent doing it to you but is a TRANSACTION between a bacterium or virus
and your immune system modulation.. So let the Doctors do their job but
you participate doing the "right thing" such as good nutrition, exercise
and most of all....peace of mind. Get busy in these 3 areas and your life experience and behavior will change.

My regards,
Dr. Gian

I JUST HAVE TO ASK IVE HAD THIS RED RASH AROUND MY GENITAL AREA
BETWEEN MY RECTUM AND MY VAGINA IVE BEEN TO THE DR AND ASKED THEM
IF THEY IT WAS HERPES AND THEY SAID NO BECAUSE THEY SAID I WOULD
HAVE LESIONS AND BE IN A LOT OF PAIN AND WOULD HAVE BLISTERS.
I GET REAL RED DOWN IN THE VAGINAL AREA BUT THAT IS IT.IVE BEEN WITH BOYFRIEND
FOR 2 YEARS AND I HAVENT BEEN WITH ANYONE SINCE WEVE BEEN TOGETHER
BUT I DID HAVE SEX WITH SOMEONE BEFORE I MET HIM MAYBE SIX MONTHS
BEFORE AND THAT WAS IT.IVE BEEN TREATED FOR YEAST INFECTION AND BACTERIAL VAGNIOSIS.
AND I THOUGHT MAYBE THAT WAS THE CAUSE BUT KNOW IM REALLY CONCERNED
HE HAS SHOWED NO SIGNS OF ANYTHING AND RIGHT NOW IM JUST SCARED
TO SAY THE LEAST COULD THIS BE SOMETHING NOT SO SERIOUS ARE SHOULD I JUST ASK FOR THE HERPES TEST TO BE CHECKED.

Hi scared,
were you checked for diabetes? Is your blood sugar OK?
It sounds like a fungal (yeast) dermatitis.
But you never know about Herpes. It could be there underlying a fungal infection.
Test for Herpes 1&2 "remote exposure" and also see you OB-Gyn and a dermatologist.
See what they think of an antifungal cream and/or a short cycle of an antiviral if you are positive for Herpes.
If is a fungus there are good medications but, remember,
both you and your partner need to work on your diet and on your intestinal bacterial flora
since that is the real source of fungus and not the genitals.
Bacterial vaginosis is an overgrowth of bacteria which normally live in the vagina (Gardnerella is the main guy!).
There again it sounds like " dysbiosis", which is medicalese for inability to live in
harmony with our friendly bacteria which have been with us for millions of years.
We have effective drugs for that also, my dear, but rather than using one bullet at a time,
it seems to me you have got to take a systemic approach and work this out from many angles
and.....don't forget the "stress part" !

Greetings,
Dr. Gian

Subject: herpes

Hi dr grian
I cant seem to pinpoint an answer to this question:
how long can herpes stay dormant for before an outbreak?
Ive heard that once you are infected you will usually have an outbreak within 30 days.
Ive been with the same partner -- only about 10 times of protected sex
and the rest oral -- for 2 and a half years.
He has not been with anyone else, either….
What are the chances that herpes has been dormant in me
for over 2 and a half years or longer (sexually active for 6 years) and also,
that I have passed it to my partner and it also has been dormant in him for these 2.5 years?
--helpme

Hi Kathy,
Picking up the virus and have clinical manifestations are to separate things.
Why? Since the process is a transaction between the virus load and your immune system
you can have a variety of forms. So you can go from nothing, or almost nothing,
to the full “Herpes show” such as general malaise, lymph node enlargement, fever, tingling sensation and redness,
small blisters which later open and leave wet ulcers which end up hardening out, crusting out and disappear.
There are however situations where the process may reaches the first, second or third stage and then withdraw
and stop and you may end up having only some tingling sensations or the appearance of a red area,
or only of a blister that once it is formed shrinks, heals and disappear.
In all this variety of manifestations the modulation of your immune system is very important and,
of course, how much participation you contribute to the show with scratching, pinching, breaking, squeezing and so on.
Now, the virus likes the epithelial cells just below the waxy part of the skin and once replicating there needs to survive
and has learned how to travel and hide. It has learned how to use the nerve pathway and  travels along those nerves
which supply the infected area until it reaches and hides within a hub called “ganglion”.
There in the ganglion it may remain sleeping {latent stage} and never appear for the rest of your life or
it may travel back to the epithelial cells of the skin and fire up again. Some people, not many,
may even have monthly break outs!!!! Two factors seem to make a difference: 1) the vigor of your
immune system and 2) whether or not you are taking the medications available for this condition.
What are the chances that the virus has been dormant in you? I can’t answer since you do not say
to have had a primary infection and since I do not yet make predictions!!!.
But you and your partner can be tested and find out whether or not you have been infected.

Hope this helps.

Dr. Gian

Dear Dr Gian,
I am sorry for the email I sent earlier and am gratefull for the information.
I've been pretty frustrated with this whole business lately not to mention hurt.
I am not given to having many partners or engaging in casual sex.
Before being intimate with this girl I dated her for a while and when it came time to be intimate
I sat down with her to discuss any relevant health concerns. When she brought this up later it was
a real slap in the face. I went to the doctor promptly all I got was an appointment card after
hours of waiting without even an explantation as to what I might be looking at.
I educated myself as best I could on Herpes, I learned about asymptomatic viral shedding and the virus's cycle.
I guess you're right, it doesn't matter. In the end you get what you get but I have an inclination for numbers.
That whole thing about how numbers can lie is nonesense. People lie. Numbers only show what is there to be seen. So anyhow as you might figure since I am confortable with numbers I felt more confortable knowing the odds. I checked some stuff with the cdc and they pretty much said the same thing you just did. There actually aren't really any solid figures on infections from asymptomatic viral shedding. Anyhow I guess that's why I wanted to know. I tried e-mailing you and several other sites for info and I guess after a week of no answers I started firing off emails. I know nothing about your job. I had no right. Take care doc.
Mark

Hi Mark,
Thanks for your note. Glad to see you more relaxed about the whole thing.
This will do good to your immune system and you interactions with the world in general,
including viruses. Get tested and send a copy of these e-mails to your partner.
Be compassionate.
May be someone lied to her also and this situation may finally shift all those involved
in a new behavior mode. About numbers.... I was struck when I was young in school to find out
that for long time we humans had only a limited numbers available to us and we did not
even have a "zero" until someone invented it somewhere in India.
When asked to describe the "zero number" he said : " IT IS REALLY NOTHING".
Happy Holidays
Dr. Gian
